Los fibratos parece que solo deben emplearse en hipertrigliceridemias severas.
En las consultas se suelen emplear cuando el paciente tiene un colesterol discretamente elevado y ademas muy elevados los triglicéridos, o al menos eso es lo que yo he visto hacer durante la resiencia y posteriormente en las consultas. Pues bien en el Drug Safety Update de Noviembre nos viene a decir que deberiamos proponer primero una estatina y solo si no logramos el objetivo utilizar un fibrato.

Advice for healthcare professionals:

• Fibrates should be considered as first-line therapy only in patients with
isolated severe hypertriglyceridaemia
• For patients with mixed hyperlipidaemia, fibrates may be used only when a
statin or other effective treatments are contraindicated or not tolerated
• In patients with primary hypercholesterolaemia, the use of gemfibrozil may be
considered, but only when a statin or other effective treatments are
contraindicated or not tolerated
• Combination therapy with a statin and a fibrate should be used with caution
and only when the benefits are expected to outweigh potential risks. Avoid
concomitant use of gemfibrozil with a statin

Por otro lado nos avisa de que la hierba de San Juan (hypericum) no solo interactua con los antidepresivos sino que tambien lo hace con los antiepilepticos. El hypericum es una hierba profusamente utilizada por nuestros pacientes como antidepresivo y en la mayoria de las ocasiones desconocemos que lo están tomando. Tambien tapiza partes de nuestros jardines como arbusto ornamental (aunque no os lo recomiendo por que tiene flores que duran un solo dia, durante muy poco tiempo al año y es bastante invasora).
